Filing Complaints

If you are experiencing difficulties with an insurance company or agent, please complete a complaint form so we can look into your case.

Complaint forms

If you are a doctor or other healthcare provider, you may submit a complaint regarding prompt payment or other service issues by completing the Medical provider electronic complaint form. Please follow all contract grievance and appeal procedures before filing a complaint with the division.

What happens when I file a complaint?

An advocate will:

  • Let you know we received your complaint
  • Give a copy of your complaint to the insurance company or agent and ask for an explanation
  • Decide what to do next based on all the information
  • Advise you of our findings

How long will it take?

Most complaints are resolved within 60 days but it depends on the type of complaint.

Confidentiality

Complaints are confidential. However, a copy will be sent to the insurance company/agent named in the complaint.
